This template provides an assessment of risk of venous thromboembolism providing guidance on high and other risk factors. There is also a risk assessment page for travel-related DVT.
The template can be located in the following ways:
- Add the template as a favourite using the F12 template launcher for quick access
- Search using the 'search features' in bottom left of SystmOne
DVT/PE Risk Assessment
Use the Assessment drop down to code a risk assessment has been performed.
Tick all relevant high risk and other risk factors applicable to the patient, noting guidance to the right. There are links to both the SystmOne BMI calculator and the Smoking template for convenience.
Impression
Highlights the risk of DVT/PE for the patient dependant on number of risk factors. Tick to code low, at risk or high risk.
Top right of the template there are links to the Wells DVT and Wells PE and PERC score templates.
DVT Risk Assessment in Travellers (For travel duration >4h)
Assessment
Tick to code a Travel-related DVT risk assessment has been completed.
Both the High risk factors and Other risk factors are free text boxes but also contain a list of presets for you to select from. Click the 'notepad' icon to the right of the boxes. Using your mouse, select the required text. Press and hold Ctrl on your keyboard to select multiple lines of text. Click Ok to add the selected text to the template.
Impression
Tick to code whether the patient is low, at risk or high risk of venous thromboembolism. Once selected, the appropriate Management guidance will automatically be highlighted in the section below.
Management
The appropriate guidance will highlight depending on the box ticked within the Impression section. Tick to document advice given. There are links to letters if needing to provide a travel cancellation report, OTC medications, compression hosiery formulary and SystmOne drug browser for prescribing (New Acute).
