To access the Possible Heart Failure template, go to Auto Consultation > Arden's CONDITIONS CARDIOVASCULAR > Heart Failure Possible.  This template can also be found on the diagnosis tab on the Heart Failure template.

The template is split between 4 tabs: Home, Consultation, Follow Up and Resources.

Home Tab

Consultation Tab

This tab is split between 3 sections: Presentation, Impression and Management.

The Presentation section allows the clinician to answer yes or no to some breathless questions along with the ankle oedema question.  There are free text boxes to add any clinical findings or concerns.  On the right of each of these boxes is an icon with a piece of paper that allows you to choose from one of the pre-sets. The far right hand side allows numerics to be recorded such as BP, Pulse, HR, RR, Sats and a few more.  There is also a link to the Vitals and Lifestyle template if required.

The Impression section gives the clinician the option to code the heart failure possibility.

The Management section has advice codes if applicable along with next steps such as ECG or Blood Tests requested, Chest X ray codes along with other management options and shortcuts to relevant next step templates on the right hand side.

Follow Up Tab

This tab allows you to record information that might have been collected from the previous consultation such as any consistent presentation to heart failure, any numeric results can be recorded and a choice from different impression codes.  The Management section allows the clinician to code any advice and referral codes along with a link to the Heart Failure Formulary and if necessary a code for emergency hospital admission and referral information.
