The Ardens Acne template can be found under Auto-Consultation - Ardens Conditions DERMATOLOGY - Acne. If you wish to use this template often, you can also add it to your F12 favourites launcher for easy access.
The template is split into 10 tabs, Home, Diagnosis, Assessment, Mild to Mod, Mod to Severe, Resolved, Relapse, Formulary, Referral and Resources.
Home Tab

Diagnosis Tab
This tab allows you to select the patients severity and atypical diagnosis. Ticking these options will add a read code to the patients record. Once this is selected, it is possible to code a diagnosis.
Assessment Tab
This tab is split into 3 sections: Assessment, Impression and Management.
The Assessment section allows you to free text the History and Examination of the consultation along with the opportunity of using the Vitals and Lifestyle template along with completing the DLQI Scored Assessment and depression screening if applicable.
The Impression section contains a dropdown box to code the patients severity with a hyperlink to the Severity Classifications webpage on the NICE website.
The Management section allows you to document the advice given using the applicable tick boxes along with treatment started tick boxes and hyperlinks to the Acne Formulary tab, the Mild/Mod tab and the mod/severe tab.
Under the consider section there are opportunities to select CHC codes, Bloods requested, referral options and a free text to type the plan moving forward. To the right of these options are shortcut buttons to the Contraception Template, CHC template if contraception discussed and Phlebotomy Template if applicable along with hyperlinks to the referral tab.
Mild to Mod and Mod to Severe Tabs
Both of these tabs follow the same pattern with the Mod to Severe including an oral antibiotic option under management.
They are split between 3 different sections; Assessment, Impression and Management.
Assessment - This allows you to complete and observations, mood, severity along with shortcuts on the right to enter any vitals, attachments and the DLQi scored assessment.
Impression - This allows you to code the patients acne diagnosis following the colours.
Management - Selecting the relevant above Impression, will then highlight the next steps advised for the patient, such as advice codes, topical treatment or antibiotics, change to treatment, contraception discussed and any follow up and referrals if required. A plan can be added at the bottom along with shortcuts to the Follow Up template and Contraception Template.
Acne Resolved / Acne Relapsed Tab
The Acne Resolved tab will allow you to record the Assessment, Impression and Management if a patients acne has been resolved through the relevant treatment.
Any Relapses can be recorded on the Acne Relapsed tab allowing any assessment, impression and management to be recorded.
Formulary Tab
This tab allows you to prescribe any relevant medication for the patient depending on the severity of the patients acne with links to any 1st or 2nd line antibiotics if applicable. The patients current medication is displayed at the bottom of the template with right click options to prescribe if necessary.
Referral Tab
The Referral tab contains various tick boxes for the severity of the patients condition. The relevant Actions next steps will light up depending on what has been selected in the referral section. The Referral Review Template is also available on the right hand side if required.
Resources Tab
This tab contains a bunch of acne resources that can be shared to the patient by either selecting the applicable hyperlink and printing the resources or using the Communication Annexe option on SystmOne to send it to the patient.
