The HMP Mental Health template can be found under Auto Consultations - ardens OTHER PROFESSIONALS - HMP Prison Template.

This template has 4 key tabs; Background, Assessment, New Diagnosis and Review.

Background Tab

In this tab it is possible to code any new Depression, Psychotic Disorder or Bipolar codes.  Please Note: To see if they have any previous codes, click on the relevant drop down box and previous codes from the list will be displayed on the right hand side.

There are also drop down boxes to record any background codes such as referral codes, Capacity, MH History, History Of Harm, Safeguarding, Smoking, Alcohol, Substance Misuse, under the care of codes and a free text box to record further information. There are options to enter further information on relevant templates to the right of each of the drop down boxes if applicable.

Assessment Tab

The Assessment tab allows you to record the type of assessment along with seen by and any free texting of History and Examination.  There are also options to record any history of harm and intent of harm along with capacity code and a tick box for a MH Physical Review with a link to the CVD Review template if applicable. There are also links to scored assessments and Vitals templates. 

Under Impression a condition code can be added from the drop down box.  If none of these codes are relevant to the patient, click on the new coded entry box to find an alternative code.

The Management section has a free text Plan box along with drop down boxes and links to relevant templates for ACCT, Goals, Leaflets, Medication, Referral, Care Plans, Crisis Plan and Follow up.

New Diagnosis Tab

This tab offers the same codes as in the New Diagnosis section on the Background Tab.

Review Tab

This section allows you to record a free text summary and who the review was discussed with.
