Home Tab


The Home page shows any outstanding Network Contract DES requirements and a summary of relevant medical history for the patient. This includes diagnosis codes, recent numerics, lifestyle information and any care plans.  Further down, any drug monitoring alerts and relevant medication summary.

Diagnosis Tab


This section allows you to add any positive/negative symptoms or if psychotic disorder is suspected.

Following diagnosis from a specialist there are QOF codes available via the drop down at the bottom to add the patient to the register. 


Review - Mental Health Tab

  • Red Stars - QOF requirements
  • Yellow Stars - Local Enhanced Services Requirements 
  • Blue Stars - National contract requirements


For clinicians requiring completing a full review, the Review pages should be used.  These pages include contract requirements and additional non-core elements providing a comprehensive review for your patient.  


Review section - Ensure the relevant Review code is added at the top of the template - these codes will allow you to audit how many patients have had their review completed.


There are presets available along side the Assessment free text box for frequently typed text, click on the preset button (notepad icon) and add 1 or multiple lines of text by pressing & holding Ctrl on the keyboard and using mouse to select. The text can be edited once added to the template.



Warwick-Edinburgh Mental Well-being Scale (SWEMWBS) 

Either enter the score directly in to the box provided or click the link to the right and there is a blue hyperlink available to the website in order to calculate. You will need to register in order to complete.

Use the entry boxes to code the scale score.




PHQ Questionnaires are available via the Depression link and 6-CIT via the Memory link



Impression section - allows you to code the patients condition and there is a link to the Mental Health Act template which provides guidance to the different sections and contains relevant documentation available as Word documents.


Record any codes for signposting or referral to IAPT - the link to the right of this is a Wellbeing Leaflet listing local services in your area (if using Ardens Plus or Pro package) or national services.


Record the appropriate Medication review codes from the drop down.  There is a link to the Ardens Anti-Psychotics Formulary.


If needed there is a link to the Glasgow Antipsychotic Side-effect Scale (GASS)



Crisis Plan

This is a Word document available for you to open & complete with the patient. Use the document to list people who the patient can contact for help, triggers that cause self-harm or suicidal thoughts, what they can do to help prevent these thoughts, list any early warning signs and how they can keep themselves safe.


Care Plan

The Mental Health Care Plan is also available as a Word document which will automatically merge through any Record Sharing, Medications, Allergies, Goals & Key Messages that are already recorded within the record.

In order for the document to pull through any new detail added during the review including the text recorded within the Assessment & Care plan boxes you will need to click 'OK' on the template or suspend. 


Once you have reopened the template you can launch the Care Plan which will pull through the latest information added.

 


Review - Physical Health Tab



Review section - add the Review code at the top of the template - these codes will allow you to audit how many patients have had their health check or declined/DNA.


Use the drop down fields to add codes relating to the labels on the left hand side.  The corresponding coding to the right of these contain advice and referral coding. 


There are various buttons embedded on the right of each field, that will support you to add additional detail.  You'll find Alcohol, Lifestyle, GPPAQ, Sexual Health, Smoking & Substance Misuse templates.


When adding numeric values i.e. weight, height, blood pressure etc. historical values will display in the yellow panel to the right of the template.  


Again there are links to other useful templates you may need during the review, this is to save you closing the current template and reopening another.


There are links to BP Monitoring, Vitals & Lifestyle and also Phlebotomy which allow you to view recent blood test results or add a new request.


Risk - SystmOne QRISK2 calculator is available and a hyperlink to calculate QRISK3.


Management - Immunisations will show all immunisations given/declined etc., also contains links to all immunisation templates to record any given during the review.


Medication Review coding available via the drop down with a link to the Drug review template and Glasgow Antipsychotic Side-effect Scale GASS.
