To access the Anxiety template select Auto-Consultation > ardens CONDITIONS MENTAL HEALTH and select Anxiety.  

This template can also be added to your F12 favourites for quick access.

The template has 6 available tabs; Home, Diagnosis, Review, Referral, Notes and Resources.

Home

The Home tab displays a summary of relevant patient medical history and any outstanding work relating to contracts/payment. Please see our Home Page Support Guide for more information.

Diagnosis

The Diagnosis tab allows you to tick any relevant symptoms the patient may be experiencing highlighting the criteria. 

A Diagnosis can be coded for either GAD, Panic Disorder, Adjustment Disorder or PTSD by ticking the relevant Diagnosis tick boxes.  These are all QOF codes.

The free text boxes also have presets alongside (notepad icon) for you to add common symptoms, click on the preset button and add 1 or multiple lines of text by pressing & holding Ctrl on the keyboard and using mouse to select. The text can be edited once added to the template.

Review

The Review Tab is split between three sections Assessment, Impression and Management.  

Assessment: The Assessment section has a tick box to code a Review (Patient reviewed 6A...)

Scored assessments such as HADS, GAD Scale, PHQ are available just below the History free text box. There is also an Anxiety Diary which can be printed, this will auto add the patients details when printed.

There is a numeric field to code BP, clicking into this field will also display any historical data in the yellow pane on the right of the template.

Alongside the drop downs to record various codes for Smoking status, Alcohol, Substance Misuse etc. there are embedded links to other useful templates to gather further information and to exclude the need for having to exit the current template and navigate to another.

Impression: The Impression section allows you to select the relevant Control code for patients condition, this also includes the Anxiety resolved code.

There are presets available along side the Plan free text box for frequently typed text, click on the preset button (notepad icon) and add 1 or multiple lines of text by pressing & holding Ctrl on the keyboard and using mouse to select. The text can be edited once added to the template.

The Resources hyperlink navigates directly to the Resources tab of the template with information that can be sent to a patient via text/email by copying & pasting the link in to Send Message.

There is a link to the Med3 & Letters template if you need to generate or view Med3 certificates.

The IAPT drop down contains signposting/referral or declined codes.

The Mental Wellbeing Leaflet is available to print as a Word document.

Medication

Links to formularies for prescribing Anxiolytics, Anti-Depressants or Hypnotics.

Referral

The Referral tab provides the criteria for documenting a referral.  

If using Ardens Pro package, relevant referral forms may be available within the Orange Arrow icon which appears in the Patient Demographics box.  For more detail please click here. 

Reports

There are registers within Reporting > Clinical Reporting > click on the Ardens Ltd Folder and select Conditions | Mental Health > Registers.
