The Dietitian Template can be found under Auto-Consultation - ardens OTHER PROFESSIONALS - Dietitian.

Note: If you regularly use the Dietitian template it may be useful to add this to your F12 Favourites for quick access.

The Dietitian Consultation template is split in three sections; Assessment, Impression and Management.

Assessment - This section has the seen by Dietitian read code tick box (the blue star indicates income is related to this field as part of an enhanced service), a reason for referral and an Assessment free text box, which has pre-set options if you click on the button.  There are also areas to record height, weight and BMI, the MUST score, a shortcut to our printable Food Diary template.  There is also a shortcut to the phlebotomy template.

Impression - This section has drop down read codes for diet, diet status and nutritional status.  A link to the Malnutrition template is also available if required.

Management - This section includes a free text box. to record any Advice / Plan notes along with Advice and Goals drop down read codes and a hyperlink to useful Diet resources on Patient.info.

At the bottom of the template there is a patient specific Dietitian Assessment word document incorporating the information gathered within the template.  Please note, for this to work please click OK and reopen the template before opening the assessment summary.
