If the patient has an existing diagnosis you can access the template via the Patient Status Alert within the Patient Demographics Box. The template will default to the review tab.
Otherwise go to Auto-consultation > ardens CONDITIONS DIABETES AND ENDOCRINOLOGY > Hypothyroidism.
The template is split in to 6 tabs: Home, Diagnoses, Review, Formulary, Notes and Resources.
Home Tab
The Home tab displays a summary of relevant patient medical history and alerts.
Diagnosis Tab
This tab is split into 3 sections; Tests, Diagnosis and Management. The Test results will be displayed at the tope of the screen. Clicking on any of the results will present the previous results on the right hand side. Depending on the information presented in the results it is possible to add the relevant hyperthyroidism code to the patients record.
The Management section depending on the type of hyperthyroidism selected will light up the next Management steps required for the patient, please see below:
Review Tab
The Review tab is split between 5 sections; Review, TFTs, Impression, Medication and Tests.
The Review section contains drop down boxes for Review, Rhythm and pregnancy along with numeric codes for BP and Pulse. Any previous values will display on right side of template
TfTs will be populated here for TSH & T4 levels in patient record, again any historical values will display on right side of template. There is a also a free text box to add any monitoring notes if applicable.
Impression - Ticking any one of the boxes in this section will then highlight the required fields in the Medication section below this. If TFTs normal is ticked for example, then the Continue thyroxine dose at field & Repeat TFTs in routine 1y tick box would be highlighted as the appropriate action and so on.
Formulary Tab
The Formulary tab allows you to prescribe and or view current & previous medications. Repeat medication can be re-issued from here and any acute medications re-prescribed.
TSH & T4 levels will be populated in the fields. Click the relevant dose required to generate the script for the medication. There are also advice codes available if required. The Ranges & guidance is displayed in the blue section.
The view in the bottom part of the template shows Allergies & Sensitivities and current & previous medications. Right click on any medication to re-issue/re-prescribe.

The Notes tab allows the clinician to compose any free text notes under the heading of History, Examination and Plan.
