If a patient is presenting symptoms of Heart Failure the template can be accessed by going to Auto consultations > Ardens CONDITIONS CARDIOVASCULAR > Heart Failure.
If the patient currently has a Heath Failure diagnosis, the template can be accessed by clicking on the heart icon under the patients demographic box.

Diagnosis Tab
The Diagnosis Tab can be used by clinicians to aid the patients Heart Failure Diagnosis.
This template is split between 2 sections: the Suspected Diagnosis section and Criteria Section. The Suspected Diagnosis section allows clinicians access to the possible heart failure template. Please see our Possible Heart Failure Support Article for more detail.
The Criteria section allows you to code the echo results following the and below the diagnosis code. It is also possible to code the LVSD, the LV ejection fraction numeric code and the echo codes. There is also a link to our echocardiogram template if required.
Please Note: The Red stars next to codes highlight that the codes in the list are QOF related.
Review Tab
If the patient requires and Heart Failure review, click on the Heart icon under the patients demographic box and click on the review tab.
This tab is split into 2 sections: Review and Impression.
The review section allows you to code that the review took place along with the applicable NYHA Code. It is also possible to code other review items such as chest pain, leg swelling or wounds and some numerics such as Pulse, BP, weight, HR as well as others. You can also use the vitals and Lifestyle template to find more if necessary.
In the Impression section you can code the patients control along with advice codes and the heart failure medication review option. There is a link to the Heart Failure Drug Formulary. For more information on how to use Drug Formularies, please refer to our Drug Formulary Support Article. Codes for Echo, exercise, IAPT, referrals and Care Plans are also available. Heart Failure Reports
Useful Ardens Heart Failure reports can be found under Clinical Reporting > Ardens Ltd > Conditions | Cardiovascular.
These reports are split into the above Sub folders and when clicked on the reports will be visible in the middle of the screen. This folder contains all useful cardiovascular reports so all Heart Failure reports will start with AF in its title.
The Case Finder reports can be used to improve your prevalence. Please refer to our Case Finder Support Article for more information.
