The template can be found on the Reception and Administration template, which is accessed by going to Auto-Consultations > ardens OTHER PROFESSIONALS > Reception & Administration or by clicking on the button on the tool bar if you have it there. It can also be added as a shortcut to F12 for quick access.

This template is useful to complete as any comments recorded from the prospective of the Chaperone may be required for future events.

The top section of the template is designed for the clinicians to complete, however these options will also be added to a clinicians template where a chaperone may be required such as the Cervical Smear template.

The bottom section of the template can be completed by the chaperone by ticking that they were present for the appointment, whether they had any concerns, what the examination was for and if the patient dressed and undressed in their presence. There is also a notes section where any other comments could be added if required. 

Don't forget to OK the template and Save the record when completed.
